Fellow Lions,

 This is the current APPLICATION FOR ASSISTANCE being used by your New Mexico Lions Eye Foundation. NONE of the old applications will be processed in the future. Please make a copy for your Club. It would also be helpful if You took the time to review the Instructions. The Application must be filled out completely and must have a sponsoring Lions Club. If You have questions during the process Foundation leadership will be happy to assist. 

The NM Lions Eye Foundation has already approved an Applicant this year for cataract surgery. 

We are here to SERVE those who demonstrate the need for assistance. The application is also on the NM D40 website.
